ChironAI OM is a pilot deployment with real patient data; this update covers feature changes only and contains no patient information.

Over the past two weeks, we delivered a substantial set of feature changes to ChironAI Occupational Medicine (OM).

Many of these improvements came directly from physician feedback, including a detailed review from the pilot's lead physician.

Below are the highlights, grouped by clinical function and explained in practical terms. Items not yet available to clinics are identified separately.

1. Doctor's First Report (DFR)

Based on the pilot's lead physician's review.

  1. Narrative DFR: Reports on the clinic's letterhead, with the official Form 5021 attached and cross-checked.
  2. Claim information: ADJ number (or “pending”), claim number, injury date, employer, carrier/TPA and attorney on page one.
  3. Body-part documentation: One confirmed list of claimed body parts, displayed on a body diagram, with a statement identifying which were examined.
  4. Vitals: US measurements, metric equivalents in brackets and automatically calculated BMI.
  5. Grip strength: Handedness and Jamar testing (three trials, five positions), with cited reference norms.
  6. Physical examination: Body-part-specific templates, left-versus-right comparison tables and AMA Guides reference ranges.
  7. Pre-signing checks: Warnings for missing, incomplete or accidentally omitted information.
  8. Amended reports: Corrections alongside the original signed DFR without altering the signed document.
  9. Diagnosis validation: Warnings for non-traumatic diagnostic codes in work injuries, plus laterality and seventh-character checks. Chiron's suggestions include supporting citations.
  10. Treatment planning: MTUS/ACOEM evidence grades, one-click RFA preparation, dated work restrictions and next-visit scheduling.
  11. Defensibility: “How would UR or the defense challenge this?” and “Defend This” reviews.
  12. Pre-visit preparation: Case briefs, identification of missing records and one-click records requests.
  13. Patient summaries: English and Spanish versions for physician approval.
  14. Report distribution: One-click delivery of signed DFRs with proof of service.
  15. Deadline tracking: Five-day filing countdown from the first examination, referencing Labor Code §6409(a).

2. Medical records

  1. Study extraction: Facility, interpreting physician, verbatim impression and clickable page citations for studies found in the records.
  2. Completeness checks: Independent identification of studies missing from the visit documentation.
  3. History of injury: AI-drafted narrative with sentence-level source citations.

3. Recording & dictation

  1. Whole-visit recording: English and Spanish recording after the physician confirms verbal consent. Audio is retained with the medical record.
  2. Rapid documentation: A draft is generated within seconds after recording stops, including examination tables, suggested diagnoses and treatment plans.
  3. Section dictation: Direct voice dictation into individual report sections.

4. Prescriptions

  1. Medication changes documented with the reason for each change.
  2. MTUS formulary status explained, with AI-flagged potential interactions explicitly identified as not a validated interaction check.
  3. CURES review documentation for Schedule II–IV medications.
  4. DEA credentials required only for controlled substances; simplified signing for non-controlled prescriptions.
  5. Physician licence, DEA and NPI information printed on prescriptions.

5. QME & medical-legal reporting

  1. Clinical recommendations: Chiron suggests findings on causation, MMI/P&S, apportionment, work restrictions and future medical care. The physician accepts, edits or rejects them.
  2. Addenda: Corrections to completed QME reports without altering the original.
  3. Finalization compliance: §4628(a) statement included, with AI disclosure when AI was used.
  4. Record-based intake: “Fill all from records” for QME intake.
  5. Version control: History and restoration of intake answers and report sections.
  6. Substantial-evidence review: Analysis of report defensibility and draft rebuttals.
  7. Worker questionnaires: Shareable English and Spanish questionnaires.

6. Coding & billing

  1. AI-assisted coding: Suggestions for E/M, procedures and medical-legal billing codes. Human confirmation remains mandatory.
  2. Missing procedure codes: Suggested codes for documented procedures, including duplicate checks.
  3. Coding quality monitoring: Accuracy reporting, first-week monitoring and alerts for failed AI calls.
  4. Late-payment calculations: Statutory penalties and interest calculations.
  5. Locality-based pricing: DWC-area pricing developed but not yet enabled.
  6. Fee schedule monitoring: Monthly checks for newly published DWC fee schedules.

7. Practice management & user experience

  1. Personal preferences controlling how Chiron drafts reports.
  2. “Report a problem” option within each report section.
  3. Version 2 visual redesign across the application.
  4. Dedicated demonstration clinic with administrator-only reset.
  5. Branded clinic communications and emails.

8. Built — not yet live for clinics

  1. Microsoft Single Sign-On: Shipped but disabled pending activation.
  2. Inbound fax: Awaiting account upgrade and assignment of individual clinic fax numbers.

9. Internal platform improvements

Not directly visible to clinics.

  1. Front Door origin verification.
  2. Microsoft Clarity restricted to non-clinical screens.
  3. Internal question channel for Eve.

10. Security, reliability & bug fixes

The items above are highlights from the feature-change work.

Additional fixes during the same period addressed:

  • Penalty display issues
  • Sign-in loops
  • Cross-site protection
  • Billing-only access controls
  • Security upgrades and reliability improvements

These fixes are not included in the feature list above.

Our objective remains unchanged: reduce the administrative burden on physicians, improve the completeness and defensibility of medical-legal documentation, and help clinics capture compensation for work properly performed.

The AI reasons. The physician decides.
